Chronic Disease Prevention Program
The mission of the Sutter County Chronic Disease Prevention Program is to improve the health
of Sutter County residents specifically in the area of chronic disease through preventive
education, and through providing disease management tools to those who are living with a
chronic condition.
None of us will escape the toll of chronic disease whether experiencing it personally or
having a family member or friend suffer from the effects. Typically chronic disease is
defined as diseases of long duration and slow progression. Heart disease, stroke, cancer,
respiratory diseases and diabetes are common chronic diseases that are causes of death
in our community.
The chronic disease prevention staff conducts presentations in the community, participates
in community events, and teaches workshops to increase awareness and educate interested
and concerned individuals in Sutter County.
Free Healthier Living Workshops
Living healthy and enjoying life means not letting chronic health conditions hold you
back. Learn how to manage your health conditions by attending a FREE
Healthier Living
workshop. These workshops were developed by Stanford University to empower people to
live healthy lives despite chronic health issues. If you have a chronic condition,
or are a spouse or caregiver of someone who does, then these workshops are for you.
These small group workshops are held once a week for six weeks for 2 ½ hours. To
